Medical Declaration Forms

Anyone that is representing Great Britain or their Home Country internationally or competing in a British Swimming ASA, SASA or WASA National event (all disciplines, excluding masters) and taking medication whether prescribed by a Doctor or purchased from a pharmacy or other retail outlet is required to declare these details to British Swimming by completing a Medication Declaration Form.  This form is available through your club via the registration/welfare officer or via the British Swimming website and must be completed annually or whenever you or your Doctor change your medication.

ASA National Age Group Championships 2007 23rd July - 26th July, Ponds Forge, Sheffield On-Line Entry On-Line Biography Entry Information Schedule of Events Conditions 800m Split Submission form This form is for use by Male swimmers who will be aged 12, 13 or 14 years on 26th July 2007 only. It will allow those swimmers to submit, for inclusion in the British Swimming Rankings, an 800m Freestyle split, taken from a Long Course 1500m swim, that has already been entered into the Ranking system.
